Mission Statement

To raise and solicit funds to provide financial support to families taking care of loved ones with Dementia, Alzheimers Disease or the frail elderly.  Our mission is to assist families to keep their loved ones at home as long as possible. Friends of Hope will provide scholarships for daycare services to help ease the burdens on families while caring for their love ones and ensure that clients have quality of life while maintaining their dignity.

 

General Purposes

The Friends of Hope Adult Day Services Corporation exists for the charitable purpose of soliciting grants/donations such that scholarships can be awarded to caregivers in need of financial assistance in acquiring adult day care services.  The awarded scholarships may be used by these qualified clients at the adult day care center of their choice. The scholarships will be awarded by the Executive Board of Friends of Hope Adult Day Services Corporation after reviewing all applicants. Scholarships will be awarded based on a review of the client’s mental and physical condition as well as financial need. Applications will be reviewed quarterly and the process will be auditable. Advertising for the applications will be carried in the local newspaper and by sending certified letters to adult day care centers in the greater Jacksonville area. 

 

                     Application Process/Criteria

Each fiscal Quarter, the Executive Board will determine the availability of funds for scholarship awards. The award will be in terms of days of care or partial cost to the recipient for a period that will not exceed 6 months. Selection will be based upon the financial need of the caregiver and the need of the client. Each applicant will provide a letter detailing such need and all documents will be maintained in a file for audit purposes. Care will be exercised to assure that family and relatives of board members will not receive scholarships. All award decisions will be recorded with adequate documentation to support the award decision.
